Office Policies and Procedures


Due to changes to our current environment,
we have new office policies we want you to be aware of
before you schedule your visit and come into our office.

Please carefully review ALL our new office policies and procedures.


Face Masks

Face masks are optional at this time. Due to our close proximity during portions of the eye exam, if you prefer to wear a face mask, the office will attempt to accommodate and provide the patient with a mask when available.


Insurance Verification

Patient agrees to provide the office with any insurance PRIOR to scheduling appointment.
Insurance issues WILL NOT be addressed AT/DURING or AFTER appointment time.
By scheduling prior to insurance verification, the patient agrees to pay the full out of pocket cost for the exam. It is the patient’s responsibility to self-submit for reimbursement, the office will not guarantee repayment .


Electronic Intake & Consents

*BOTH new and established patients need to complete the intake form/paperwork & new consents.

Patient will be provided a link to the paperwork and consents via email and/or text message.
This paperwork can be completed on any laptop or mobile device and does not require downloading any software or applications.

*Should you not have an email address on file and/or do not have a smartphone mobile number on file, please click on the link at the top of the page, or here, to print and complete the intake forms and consents.

All paperwork MUST BE COMPLETED IN FULL BEFORE arrival for appointment. 


Ultra-Wide Field Retinal Imaging

Advanced ultra-wide field retinal imaging is now a required standard of care at our practice. UWF retinal imaging provides an extremely valuable adjunct to the diagnosis and monitoring of retinal pathology but it is not a replacement for dilation. Many insurances do not cover the cost of this retinal scan and the fee will be in addition to your routine eye exam copays.


Illness

If you and/or your family members are sick and/or have the flu or COVID19, please call and reschedule your appointment. The office reserves the right to reschedule anyone exhibiting cold symptoms including but not limited to: fever, coughing, sneezing, etc. 


Failure to show, Late Arrival,
Late Cancellation Policy

Out of respect for everyone’s time, we have an established cancellation policy. 

Late arrivals: The office will not be able to accommodate any patient arriving later than 10 minutes from their scheduled exam time; he/she will need to reschedule.

Late Cancellations: The office requires at least 24 hours notice to cancel or reschedule an appointment.  

*** Failure to show up for an exam, late arrivals & late cancellation will
ALL result in a $35 late cancellation fee. ***
